Sore Throat vs. Strep Throat in Kids: When to Seek Care

“Mom, my throat hurts!”

Every parent has heard this phrase more times than they can remember, because a scratchy or painful throat is one of the most common complaints kids bring to their parents. (And it rarely means just one thing.)

Colds, seasonal allergies, dry indoor air, and a variety of viruses can all leave a child’s throat feeling raw. Strep throat is just one possibility among many. The tricky part for parents is that strep often looks similar to these other, milder causes in the first day or two, which is why it can be hard to tell what you are actually dealing with.

Here’s what you need to know about figuring out what kind of sore throat your child has and what you can do about it.

What Are Possible Signs of Strep Throat?

While there is no single symptom that confirms strep on its own, certain patterns tend to show up together more often with a strep infection than with a typical cold:

  • A sore throat that develops quickly, sometimes overnight
  • Fever
  • Swollen or tender glands in the neck
  • Noticeable pain when swallowing
  • Little to no cough, which is unusual for a cold or virus
  • Stomach pain or headache in some children

Symptoms like a runny nose, cough, hoarseness, or red, watery eyes often point more toward a virus than strep throat.

None of these signs on their own mean your child definitely has strep, and plenty of viral sore throats share some of these same features. That overlap is exactly why testing, rather than guessing, tends to give families the clearest answer.

Testing helps your provider decide the right next steps, whether that means starting an antibiotic for a bacterial infection or focusing on comfort measures while a virus runs its course. Skipping straight to a guess, in either direction, can mean either unnecessary medication or a missed infection that needs treatment.

When Should You Go to the ER?

Most sore throats, strep included, can be handled with a same day visit. However, a small number of cases need emergency attention. 

Head to the ER at once if your child:

  • Is having difficulty breathing
  • Is drooling more than usual because swallowing has become too painful
  • Cannot swallow liquids at all
  • Shows signs of severe dehydration
  • Has severe one-sided throat pain or trouble opening their mouth

These symptoms are uncommon, but they should never be ignored.
